A 49-year-old man with ventricular pre-excitation (Figure 1) underwent electrophysiological study. A narrow-QRS tachycardia with alternating cycle length and QRS morphology was induced (Figure 2). What is the tachycardia mechanism? What are the different causes of alternating cycle length during supraventricular tachycardia? Is QRS alternans a diagnostic clue?
